Schuyler Hospital Now Offering Patient Portal

This fall, Schuyler Hospital and their clinics began offering a “Patient Portal” – an opt-in opportunity for patients to access personal health information on-line. Patients can register when they visit their primary care physician, or when they are at the hospital for an in-patient stay or Emergency Department visit. Accessible information varies, but may include details of the patient’s most recent visits, including blood pressure, heart rate, health care provider’s care instructions, medication list, and more. All information is provided in a secure record available only to the patient and hospital care team. Those who have signed up with their primary care provider may also use the Portal to request an appointment with their provider – though this is not intended for same day or emergent needs. The service is free to patients, and offered through RelayHealth. Additional tools and communication services may be added at a later date. Interested?1.
